Producer Application - Processed Food
Thank you for your interest in becoming a producer member of the Oklahoma Food Coop. A Processed Food Product is a food that requires a Processing Authority Letter issued by an agency that issues processing authority letters to be legal for sale. This does not include processed meat products like sausages, meats, charcuterie prepared by a USDA or ODA licensed processing facility. If you have a question as to whether your product requires a Processing Authority Letter, please contact the appropriate health department officials.

This application is only for Processed Food Products that require Processing Authority Letters. There is a separate application for prepared or processed products that do not require a Processing Authority Letter. If your answers are short, terse, and insufficiently informative, your application processing will be delayed because we will return it to you to ask for more complete information. Before submitting this application make sure you have read and understand and agree to the cooperatives Terms of Service.

Section 1: Products and Practices
1. List each processed food you plan to sell through the Oklahoma Food Coop.

For each and every processed food product you plan to sell through the Coop, you must:
  • List all of the ingredients. If you use any mixes, they must be identified as such and all of their ingredients included in the ingredient list.
  • Note which ingredients, if any, are of Oklahoma origin.
  • Note any ingredients that are Certified Organic or Certified Naturally Grown.
  • List whether the product is delivered at room temperature, frozen or refrigerated.
  • List the processing authority letter number for each product and the agency which issued the number(s).
2. If meat or eggs are ingredients in any of your products, list the non-CAFO Oklahoma source of those ingredients.
Our rules forbid using CAFO originated meats or eggs in products sold through the Oklahoma Food Cooperative, and require that meat or egg ingredients must be acquired from sources within the state of Oklahoma.
3. List any allergens in your products. Does your product labeling alert the customer to potential allergens?
4. If any of your products are frozen or refrigerated, how will they be transported to the coop's operations center so that they arrive at the proper temperatures for storage?
5. How will your products be packaged for distribution through the coop?
6. List the inspecting agency for your kitchen/food facility, including the date, name, and contact info for the most recent government health inspector.
7. Describe any sustainability, energy conservation, or other miscellaneous "green" aspects of your business.
8. Describe any training or classes you have gone through to help you learn how to do what you do, including your training in food safety and production.
9. Have you identified the potential risks involved in your food production processes and developed plans to mitigate/avoid those risks? If so, briefly describe your plans to secure the food safety of your products. If not, when do you plan to do this?
10. Please tell us your story. Write a couple of paragraphs about who you are, why you are doing this, how participation in the Coop will benefit you, etc.
