Producer Application - Food
Thank you for your interest in becoming a producer member of the Oklahoma Food Coop. Oklahoma Food Coop customers and producers are interested in local foods and products produced with sustainable practices that demonstrate good stewardship of the environment. Upon approval, this form will register you to sell products within the Oklahoma Food Coop. Please complete and submit the form below.

This application cannot be used for approving producers selling processed or prepared foods (except for cured/processed meats produced by a USDA or ODA inspected livestock abattoir). There are separate applications for producers intending to sell those products.

All producer applicants must answer all of the questions in section 1. All producer applicants intending to sell meat or poultry must also answer all of the questions in section 2. It is better to say more rather than less about your production practices. If your answers are short, terse, and insufficiently informative, your application processing will be delayed because we will return it to you to ask for more complete information. Section 1: Products and Practices
1. List what you intend to sell through the Oklahoma Food Coop. (e.g. meats, grains, vegetables, poultry; also note if you have any heritage breeds)
2. Describe your production practices.
3. Do you use genetically modified seeds or plants?  YES     NO
4. Describe your pest and disease management system.
5. Describe your soil and nutrient management. (Do you compost, use fertilizers, green manures or animal manures on your fields, gardens or pastures?)
6. Describe your water usage practices. (If you irrigate, describe how - e.g. deep well, surface water, etc., and explain how you conserve water or use best management practices. Describe how you are protecting your water sources from contamination/erosion. What happens with water run-off from your property?)
7. Describe your conservation/land stewardship practices. (e.g., do you plant windbreaks, maintain grass waterways, riparian buffers, use green manures for wind erosion, plant habitats for birds, improve soil quality, etc.)
8. Describe any additional information and/or sustainable practices about your operation that would be helpful to a potential customer in understanding your farm or operation better. (e.g. if you are raising any heritage animals you might list breeds or list varieties or heirloom seeds.)
9. List your food liability insurance coverage, both general and product-related, as well as any licenses and test that you have available.
10. Where is your operation located? (You must give the exact location of any fields or pastures from which products that will be sold through the Oklahoma Food Coop originate, as well as the legal mailing address of your farm/ranch.)
Section 2: For Meat and Poultry Producers
1. Describe your herd/flock health and productivity management. (i.e. do you use any hormones, antibiotics, and/or steroids. What happens with sick animals or birds, etc.)
2. Describe your feeding practices. (i.e. do you grass/forage-fed only, free-range, grain-fed, feed-lot, etc.)
